• verifiable employment,

• own your vehicle,

• have a current registration, and

• have insurance,

complete the following form and submit it to:   Wheels4Working@gmail.com

* Note: in order to receive assistance, applicant must be working a minimum of 20 hours per week and have been employed for at least 90 days.
